As part of Cleveland Clinic’s commitment to population health and in support of its Accountable Care Organization (ACO) (https://my.clevelandclinic.org/patients/information/aco), these ACO measures have been prioritized for monitoring and improvement. Cleveland Clinic is improving performance in these measures through enhanced care coordination; ensuring patients receive the appropriate care at the right time; optimizing technology and information systems; and engaging primary care specialty teams directly in the improvement work. This collaborative improvement work part of Cleveland Clinic’s overall strategy to transform care in order to improve health and make care more affordable.
